Improper Humidifier Use: Why It Could Increase the Risk of Lung Infections

When the weather turns dry, many households run humidifiers for extended periods. However, if the water tank is left uncleaned, old water is continuously topped up, or the indoor humidity is set too high, a humidifier can become more than just a source of moisture—it can become a breeding ground for microbes and mold.

It is important to clarify that humidifiers do not magically "create pneumonia," and using one correctly does not guarantee an infection. The real risks to watch out for are contaminated equipment, improper additives, and excessive, prolonged humidification.

Why Can Improper Humidifier Use Harm the Respiratory System?

1. Water Tanks Can Become Microbial "Petri Dishes"
The water tank, base, and misting channels of a humidifier remain damp for long periods. If water is merely topped up rather than replaced, or if scale and biofilms are not regularly removed, bacteria and fungi can multiply. When the device is turned on, these microbes and their byproducts can be carried into the indoor air via fine water droplets and inhaled. This exposure is particularly concerning for the elderly, infants, individuals with chronic respiratory diseases, and those with weakened immune systems.

The U.S. Environmental Protection Agency (EPA) notes that portable ultrasonic and impeller humidifiers can disperse microbes and minerals from the tank into indoor air. Therefore, they recommend changing the water daily and cleaning the device regularly.

2. Excessive Humidity Promotes Mold and Dust Mite Growth
Air isn't always better when it's wetter. Continuous over-humidification can cause moisture buildup in corners, on curtains, mattresses, and inside air conditioners, creating an ideal environment for mold and dust mites. Mold spores and related allergens can trigger stuffy noses, coughing, wheezing, or allergic reactions, and may also exacerbate existing conditions like asthma. The EPA recommends keeping indoor relative humidity between 30% and 50%, ideally not exceeding 50%. If you notice condensation on windows, damp walls, or water droplets on furniture, you should immediately reduce the humidification level.

3. Minerals in Tap Water Can Create "White Dust"
Ultrasonic humidifiers atomize water directly. If tap water with high mineral content is used, substances like calcium and magnesium can be dispersed into the air along with the mist, eventually settling on furniture as a white powder. A portion of this can also be inhaled. While these minerals are not pathogenic and do not directly cause pneumonia, they do increase the particulate burden in the air. The EPA recommends using low-mineral water, such as distilled water, and replacing demineralization cartridges as instructed.

4. Randomly Adding Disinfectants, Essential Oils, or Medications Can Directly Irritate the Lungs
Unless the product manual explicitly permits it, never add disinfectants, sterilizers, perfumes, essential oils, herbal medicines, or nebulized drugs to the water tank. Just because a substance is safe for skin contact or surface cleaning does not mean it is safe to inhale into the lungs. Atomized chemicals can irritate the respiratory tract and, in severe cases, cause chemical lung injury. This type of damage is entirely different from infectious pneumonia caused by bacteria or viruses and should not be confused with it.

"Humidifier Lung" Is Not Always a True Infection
The internet often lumps lung issues following humidifier use under the umbrella term "humidifier pneumonia," but medically, this could involve several different conditions:

  • Contaminated mist increases microbial exposure, raising the risk of infection.

  • Mold, bacterial components, or other antigens cause hypersensitivity pneumonitis (an allergic reaction).

  • Chemicals like disinfectants cause aspiration or chemical lung injury.

  • Pre-existing asthma, COPD, or allergic diseases are aggravated by irritation.

Therefore, you cannot self-diagnose based solely on the combination of "using a humidifier + developing a cough."

How to Use a Humidifier Safely

1. Monitor Humidity Before Turning It On
Use a hygrometer to keep indoor relative humidity between 30% and 50%. If the air is already comfortable, there is no need to continue humidifying. If you notice condensation on glass, damp walls, damp bedding, or a worsening musty smell, stop using the device and ventilate the room to reduce moisture.

2. Empty and Refill with Fresh Water Daily
Do not simply top up yesterday's leftover water. Every day before use, pour out the remaining water, rinse the tank and base briefly, and add fresh water. When not in use, empty the internal water completely and keep the device dry.

3. Clean Thoroughly and Regularly According to Instructions
The EPA recommends cleaning portable humidifiers at least every three days; if the manufacturer's instructions require more frequent cleaning, follow their guidelines. When cleaning:

  • Always unplug the device before disassembling.

  • Remove scale, biofilms, and visible dirt.

  • Use the cleaning methods recommended by the manufacturer.

  • Rinse thoroughly after using any cleaners or disinfectants to prevent residue from being atomized.

  • Replace filters promptly when they expire.

4. Prefer Low-Mineral Water
For ultrasonic humidifiers, use distilled or low-mineral water to reduce white dust. Note that purified, filtered, and distilled water are not entirely identical; choose based on your local water quality and the device's manual.

5. Do Not Blow Mist Directly at Faces or Beds
Keep the humidifier at a safe distance from beds, sofas, and walls. Do not allow the mist to blow continuously toward the elderly, children, or sleeping individuals, and avoid placing it directly against wooden furniture, electronics, or walls.

6. Stop Use and Seek Medical Attention if Symptoms Occur
If you repeatedly experience or notice a worsening of the following symptoms after using a humidifier, stop using the device immediately, open windows for ventilation, and seek medical care:

  • Persistent cough or fever.

  • Chest tightness, shortness of breath, or wheezing.

  • Chest pain or severe fatigue.

  • Drop in blood oxygen levels.

  • Changes in mental state, lethargy, or significant loss of appetite in the elderly.

When seeing a doctor, proactively inform them about the duration of humidifier use, the type of water used, cleaning frequency, and whether any other substances were added to the tank. If elderly individuals experience severe breathing difficulties or altered consciousness, seek emergency medical help immediately.
